The purpose of this policy is to help you understand the items of the usage policy when you use the National Patient Safety Platform (hereinafter referred to as "the platform" or "the national platform") and share your data as a user on the internet, and how the platform handles it.

• The platform places the confidentiality of its users' and visitors' information among its top priorities, and the platform's management makes every effort to provide high quality service to all beneficiaries.
• The users of the platform shall continuously review the privacy policy and the usage policy to be informed about any update, and your usage of the platform shall be considered your knowledge and agreement to those policies.
• The user of the platform is responsible for the completion and validity of the information he sends through the platform.
• To keep your personal information, the electronic storage and the sent information are provided by using the secure technologies compatible with the regulations and the policies of the National Cybersecurity Authority
• The external links existed on the platform is for reference purposes and the platform is not responsible for the content of the external websites.

Definition:

The National Patient Safety Platform is a unified electronic platform to monitor events, medical errors and their consequences when healthcare services are provided. It analyzes the root causes, studies them, finds appropriate solutions and prepares corrective plans for them.
The platform is available for use by healthcare providers or healthcare facilities and clusters to enable them report and monitor sentinel events and medication errors, measure patient safety culture among healthcare facilities, projects, different programs that reduce and prevent medical damages.
On the platform, the recommendations, reports, alerts, scientific sources, policies and guidelines concerned about patient safety are publish, which in turn, enhance patients, visitors, and healthcare facilities' workers' safety and security by raising the culture of patient safety, reducing and preventing reoccurrence of damages.
The National Platform maintains confidentiality of the data and information entered into it, and aims at gathering and analyzing the data, analyzing reasons behind events and reports and finds corrective plans, and then, publishes recommendations and reports to raise the culture of patient safety, reduces and prevents reoccurrence of damages.
Therefore, usage and access to the platform are subject to the terms and conditions of use of the Kingdom of Saudi Arabia's laws. Therefore, registration and access to the platform are considered as an agreement without any restriction or condition related to the terms and conditions of use. It is also considered as an agreement to deal electronically with the Saudi Patient Safety Center and comply with all legal consequences related to that whether the user is registered or not.
When using the platform for the first time, the agreement shall enter into force, and its terms conditions will apply directly to the users who shall comply with the terms and conditions of use, whether they are healthcare providers or healthcare facilities and clusters of the platform or who represent or act on behalf of them, and who is authorized to use the platform and the available services in any way.
Any amendment of these terms and conditions shall be immediately effective once it is announced on the platform, unless otherwise indicated.

Information Policy:

• The user must verify and acknowledge to provide accurate, updated and complete information.
• The user must not provide any information or any sensitive personal information including but not limited to, (a patient's name or file, and a healthcare provider's name or employment number).
• The user must maintain the information and update it continuously if requested.
• The user must be responsible for any actions taken in the event of refusal to comply with the forementioned points.

Restrictions of Usage and User Policy:

• Bear full responsibility for keeping the password of the account and not disclose it to any party.
• The user is responsible for the data and information entered into the platform and its effects, and the user is obligated to provide accurate, correct, real, complete and up-to-date information about the data and information required in accordance with the requirements of all operations of using the platform
• The user is responsible for alerts and notifications received by him via the platform and is considered informed and notified as of the date they are sent to him.
• Maintain the confidentiality of the available information about the platform and the users.
• The user acknowledges that all the data provided is true and real, and the platform is not responsible for any error contained in it.
• The user acknowledges to refrain from misusing the platform in any way, such as sending, publishing, advertising, distributing, or circulating contents, materials or information aimed at distorting or violating laws.
• Acknowledge consent and knowledge that all information and received data will be shared with the employees of the Saudi Patient Safety Center concerned with the information or the data for the purpose of analyzing, processing and studying it to find appropriate solutions to enhance the safety and quality of the medical system.
• Request to update the information by sending an information update form through the platform to constantly ensure the validity and accuracy of it because invalidity or inaccuracy of the information registered on the platform by the user will suspend or delete the user account and make the user unable to access his account or use the electronic services provided by the platform.
• Acknowledge to refrain from uploading any files on the platform that have any sensitive personal information including but not limited to, (a patient's name or file, and a healthcare provider's name or employment number), corrupted data, viruses or malicious files with or without knowledge.
• Acknowledge to refrain from uploading any files on the platform that have software or suspicious materials or data or other information that is not owned or for which you do not have a license.
• Refrain from using the platform to contribute illegal activities inside or outside the Kingdom of Saudi Arabia.
• Refrain from advertising any product or service that make the platform violate any law or regulation applicable in any field or site.
• Refrain from using any means, program or taking a procedure to intercept or attempt to intercept the proper operation of the platform.
• Refrain from taking any procedure that imposes an unreasonable load (or uploading big files in an inappropriate way) on the infrastructure of the platform or negatively affects the infrastructure or its content or publishes, announces, distributes or circulates materials or suspicious information that ruin reputation, violate laws, or that is abusive or contrary to public morality, as well as any materials or information that is illegal on the platform.

Rights of the Saudi Patient Safety Center:

• The Saudi Patient Safety Center has the right to contact the reporter once the report is submitted and received for further information, data or clarification.
• The Saudi Patient Safety Center reserves the right to change the type of the event or the medication error or the level of the damage according to the description submitted by the reporter to ensure the validity of the data analysis in accordance with the Saudi Healthcare Sentinel Events Manual.
• The Saudi Patient Safety Center has the right to use the unidentified data for educational purposes.
• The Saudi Patient Safety Center has the right to change the services of the platform that are not included in this document without any prior notification.
• The Saudi Patient Safety Center reserves the right to suspend or cancel the account of the user in case of violating any condition of this document.
• The Saudi Patient Safety Center reserves the right to cancel the user's account access to the service if the user consumption of the platform exceeds the terms and conditions of using this service.

Guarantee of Rights of Use and Accuracy of Information:

The user is obligated to ensure the validity and accuracy of the information and data he provided and entered into the platform that may affect the objective of the service or the final output. The Saudi Patient Safety Center is not responsible for any incorrect or inaccurate data or information provided or entered into the platform that may affect the objective of the service or the final output.

Links and Sources of The Platform:

• In order to easy access the platform, links and QR codes (barcodes) are provided. Therefore, the user must acknowledge to refrain from misusing the links or the barcodes in any way including sending, publishing, announcing distributing or circulating any material that is misusing or violating the objective or the purpose of them which considered as a violations of the laws.
• The platform has scientific sources, references recommendations, reports, alerts, policies and guidelines that are concerned with patient safety and enhance the safety and quality of healthcare, the Saudi Patient Safety Center is responsible for updating these materials constantly.
• If there are links to websites or portals not within the platform including but limited to, scientific references and sources, the user will be responsible for accessing these links. The Center is concerned with replacing and updating the electronic links and references that are not working on the other websites, but does not ensure they will always work.
• This platform may contain electronic links to websites and other platforms that use ways to protect their information and its privacy that may differ form that the platform uses. We will not be responsible for the content and ways other websites use for privacy, and we recommend to refer to the privacy policy they apply.

Protection from Viruses:

Efforts are exerted to investigate and test data and content to protect the platform from any effect, and we always recommend to activate an up-to-date version of anti-virus program for all materials downloaded from the internet. The Center is not responsible for any loss, interruption, corruption of your data or computer that may occur when connecting to the platform or use its services or materials.

Browsing:

We have not designed this platform to gather your personal information from your computer when browsing this platform, but will use the data you provide.

Internet Protocol (IP)

Anytime you visit the platform, the host server will register your internet protocol (IP), date and time of the visit, the type of the internet browser you are using, and the URL of any internet website that redirects you to the platform on the network.

Surveys and feedback:

The surveys and sharing of surveys that we carry out directly on the network enable the platform to collect specific data such as the data required of you regarding your experience of the platform, and your responses are of utmost importance and appreciated by us as they enable us to improve the level of the platform, and you have full freedom and choice to provide data related to your name and other data.

Waiver of Claims:

• The National Patient Safety Platform is an electronic platform that is used by healthcare providers or healthcare facilities and clusters to enable them report and monitor sentinel events and medication errors, measure patient safety culture, sources and materials. The services can be accessed and used the way they are available on the platform.
• The Saudi Patient Safety Center is not responsible for any abuses or errors result in data submission or acknowledging their accuracy and not containing private personal information with or without knowledge.
• The Saudi Patient Safety Center does not guarantee or bear any interruption, abuses, or error that may arise when using the platform, links, or materials, whether with or without knowledge.
• Any information that the user exchanges, shares and sends through the platform including data, documents and survey databases related to patient safety, the user acknowledges the approval of sharing intellectual property rights with the center.

Limitation of liability:

• The user agrees not to use, encourage, promote, facilitate, or instruct others to use the services as follows:
• Unauthorized access or investigation of any service or system, including, but not limited to, violation, vulnerability scans, or breach testing.
• Disable, interfere with, or circumvent any aspect of the service; or violate any security or authentication measures used by the system or the service.
• Any theft of resources including sensitive information.
• Services electronically provided by the platform, and access to those services require the use of the Internet, and therefore, the user acknowledges that using the platform is at his own responsibility and that communication via the Internet may be subject to interference or objection by others, and the Center is not responsible for any consequences that may occur when using the Internet to access the platform.

Compensation:

The Saudi Patient Safety Center is not responsible for any claim that arises when using the platform, publishing or responding individually to the events or errors submitted and reported via the platform, or paying any financial rewards to the reporter.

Termination of Use:

The Saudi Patient Safety Center has the right, without prior notice and at its discretion, to terminate, restrict, delete or suspend the user’s account, due to violation of the terms and conditions of use or any illegal action or behavior that is contrary and harmful to others. In the event that the account is suspended, he will not be authorized to use the platform, and the Center has the right in such cases to legally refer to the offender.

Intellectual Property Rights:

• This platform is owned and supervised by the Saudi Patient Safety Center, and all materials, services and contents are protected by copyright and intellectual property rights.
• The beneficiaries may not use the materials and content available on the platform other than what they were intended for, and any modification contrary to the main purpose for which they were allocated is strictly prohibited, without obtaining the prior written permission and consent of the center.

Dispute Resolution:

In the event of claims and disputes that may arise from using the platform, the user agrees to be subject to the judicial authorities in the Kingdom of Saudi Arabia.

General Terms:

The Saudi Patient Safety Center has the right to disable any unauthorized link, and the center is not responsible for any information provided by users that is unreal, inaccurate, or contains information that violates confidential privacy rights. These terms and conditions and the usage policy are subject to the regulations in force in the Kingdom of Saudi Arabia, and they are interpreted according to those regulations.
